Dose dates materially change what is due: the next dose can't precede the last recorded dose by less than the ACIP minimum interval, so a recent dose pushes the next due date out. Leave dates blank if unknown (the engine falls back to "earliest possible from today"). Leave the whole section blank to assume zero doses — the CDC default for undocumented arrivals without written records.
